    ONLINE RESERVATION SYSTEM - 
AGREEMENT & BUSINESS INFORMATION DATA ENTRY FORMS       

To participate in the online reservation system, you will need to complete TWO forms.  If either form is incomplete, your business will not be represented on the booking system until completed forms are received. 

FORM # 1 - AGREEMENT FORM

Please choose the appropriate agreement form from the following: 

• Accommodation Provider - Agreement Form
  
• Restaurant - Agreement Form
  
• Attraction/Retail - Agreement Form

FORM # 2 - DATA ENTRY FORM

The information contained in this form will be used to complete your company's microsite on the Online Reservation System. 

Please download the appropriate form below and complete the required information. 

Both PDF and Word Documents are available. If you use the PDF version, you will need to print the document and enter information by hand.  If you are entering information directly into a Word Doc, YOU MUST SAVE A COPY OF THE FORM before sending. 

Completed forms can be submitted directly to Meridian - instructions are included in the form.
